Welcome, if interested in becoming a Preferred Specialist with our office please complete our Direct Appointment Referral Program Form to participate. Our office we will be in touch shortly.
Program Guidelines
Be advised, our office reserves the right to enlist providers who meet our recommendations for intervention. By your participation, this does not guarantee or grant you access to be placed on our list with every client/patient service. When appropriate, the client/patient will contact your office directly to schedule an appointment.
COMMITTED We hold true to our word, and we expect the same from our Preferred Specialists. After multiple unsuccessful client/patient interactions including scheduling attempts, our office will no longer partner with the organization, therefore ending all agreements of our Direct Appointment Referral Program. Such measures will be clarified from our office to resolve the concerns of the agreement before discontinuing would take effect.
We are aware that every organization differs in terms of services offered and availability;
(1) we know that not every client/patient will be best suited for the services offered and (2) available options for a scheduling date within two weeks are not always feasible/beneficial to the client/patient in a timely manner, and should be informed to the client/patient upfront to make sure they received the best care possible.
