Rescheduling

Overview

After contacting the department your disability case examiner will schedule a formal request for an evaluation through a licensed professional contracted with the department. Once a date and time has been finalized in the system, the department will send a reminder notice informing the parent/guardian(s) to contact the provider of the evaluation to confirm your scheduled appointment.

 

Who We Are

Psych Assessment Resources (PAR) encompasses a Bilingual Licensed Psychologist (Spanish/English) who has extensive training backgrounds, offering a variety of psychological services ranging from Psychoeducational and Forensic Evaluations to Autism Spectrum Disorder and Individual/Group Therapy. Inclusive of community diversity, PAR is a premier Mental Health and Wellness Practice that focuses on individuals across the entire lifespan (Infants, Children, Adolescents, and Adults).

Lead by Dr. Sonia G. Perala, Bilingual Arizona-Licensed Psychologist and Certified Therapist with over 25+ years in the field of psychology. Dr. Perala's expertise is focused on Child/Adolescent Development with emphasis on the Special Education Process (504 Plan/MET/IEP), Psychological Assessments and Evaluations (Infant - Adult), Attention Deficit Hyperactive Disorder, Autism Spectrum Disorder, Bonding/Best Interest Assessments, Intake Interviews/Record Reviews/Behavioral Questionnaires, Counseling and Play Therapy.

Appointment

LENGTH 2 - 4 Hours (Depended Upon Child’s Performance)

DAYS Tuesdays & Thursdays (By Appointment Only)

TIMES 8:30AM / 11:30AM / 1:30PM (It’s Best To Arrive On Time, No Earlier Than 10 Minutes)

 

Location

Depending on the area location of services being requested, the department will advise you of which office you will be attending your appointment. In most cases, the department will notify you in advance with a mailed confirmation of your appointment. If you have further questions, please contact the appropriate DES office.

 

Requirements

• Parent/Guardian(s) please bring a valid form of identification - Driver’s License/Passport/Birth Certificate/SSN Card
• Prepare any necessary items your child may need before the appointment - Prescribed Medications/Required Glasses/Hearing Aids
• Have snacks and water available for your child as they may need a short break during the appointment

 

Testing

During the evaluation your child will be given certain testing assessments depended upon the department, typically these directives are taken from parent/guardian’s request for services. We ask parent/guardian(s) to fill out background history forms/questionnaires for a complete evaluation. We require the child’s full cooperation and performance abilities during the evaluation, as well as, the parent/guardian’s transparency toward information being requested by the elevator. If the department does not get a complete picture of the evaluation it may delay the decision of your claim and/or possibly be denied.

Copy

EVALUATION REPORT Parent/Guardian(s) may request a copy of the written report directly from the department. PAR cannot provide a copy directly and will advise to contact your Disability Examiner for the complete report.
